I hereby give American Eskimo Rescue and Sanctuary of Iowa the authority to verify my veterinary references and to perform a home visit if necessary. I understand that American Eskimo Rescue and Sanctuary of Iowa reserves the right to refuse adoption if information provided is erroneous, incomplete or fictitious.
 I also understand that by visiting the American Eskimo Rescue and Sanctuary of Iowa, I agree to be solely responsible for any injuries or mishaps as pursuant to Iowa domesticated animal code 673 (listed below) By clicking the send button I am verifying that I have read these terms and are agreeing to these terms.
